Health Choices and Planning
Advance care planning for any age
and any state of health.
Most people know what matters to them. Far fewer have written it down, named a decision-maker, or told their family. That gap is where crisis planning happens — in hospital hallways, under pressure, by people who are guessing.
Avoidance is not an advance directive.
I help you close the gap while things are calm. We work at your pace, in plain language, until your wishes are documented, your decision-maker is prepared, and your family has actually heard you say it.

What We Work On Together
-
Advance directives. Documenting your healthcare preferences so they’re honored when you can’t voice them yourself.
-
Choosing your healthcare surrogate. Selecting the right person to make decisions on your behalf — and preparing them for what the role actually asks.
-
A personal care plan. A written plan that reflects your values, not a template’s.
-
Documenting treatment preferences. Specific enough that healthcare providers can follow them.
-
Organizing important documents and your digital legacy. Where everything lives, who can access it, and what happens to your online accounts.
-
Family conversations. Conversation starters, and facilitated family meetings when you want a steady third party in the room, so everyone hears the same thing at the same time.
I don’t provide legal or medical advice. I help you get clear, get it in writing, and get everyone on the same page — and I’ll tell you when a question belongs with an attorney or your medical team.
